Overview
The Journal Page Counseling LLC respects the privacy of client health information. Our Notice of Privacy Practices explains:
• How health information may be used and disclosed.
• The rights clients have regarding their information.
• The Practice’s legal duties concerning privacy and security.
• How clients may request records, corrections, restrictions, or confidential communications.
• How clients may file a privacy complaint.

Privacy questions or concerns may be submitted to the Practice Privacy Officer. The Journal Page Counseling LLC will not retaliate against anyone for making a good-faith privacy complaint.
Texas Medical-Privacy Complaints
The Texas Attorney General is not the licensing board for professional counselors. Professional-conduct complaints should be directed to the Texas Behavioral Health Executive Council.

Federal HIPAA Complaints
